TOPCARE: Implementation of a telematic homecare platform in co-operative health care providr networks.

The overall objective of TOPCARE is to develop technical devices and telecommunication structures and to lay the organisational groundwork for bringing cooperative health care services into the home of the patient.

Continuity of care will be achieved by integrating the home based services into a network of health care providers. It is the objective in TOPCARE to generate new types of remote but safe patient care concepts. The proposed innovative telematic care concept in TOPCARE holds the promise to serve as a modular, standardised homecare platform with a single entry point for caregiver networks.

The platform approach will overcome problems due to incompatibilities between home monitoring equipment, remote monitoring software and health information systems. The TOPCARE platform will provide secure and confidential patient data communication and will support co-operative caregiver networks.
